AGREEMENT FOR BMH LOGISTIC LLC 

1. Recitals

This agreement between  BMH Logistic LLC and M AND R Enterprise Trucking LLC hereinafter referred to as ‘Client’, desires to retain Do Trucking Solutions LLC to find and secure freight for Client and dispatch Client's equipment. Prior to the implementation of this agreement, Client must furnish to BMH Logistic LLC the following documents for smooth operations:

1. This Agreement form completed and signed

2. Copy of Client's Operating Authority.

3. A signed W-9.

4. Proof of Insurance Certificates

5. Notice of assignment (If setup with Factoring) or a voided Check

5. Payment Authorization forms

2.    Communication Agreement with Carriers/Drivers:

By engaging in business with BMH Logistic LLC, carriers and drivers hereby acknowledge and agree that they may be contacted via phone calls, text messages, or other forms of communication at any time and as frequently as necessary for the purpose of coordinating load bookings and related logistics. This includes but is not limited to inquiries about availability, scheduling, pickup/delivery instructions, and any other pertinent information relevant to the transportation services provided by BMH Logistic LLC. Carriers and drivers consent to receiving such communications as an essential aspect of their engagement with BMH Logistic LLC and acknowledge that prompt and efficient communication is integral to the successful execution of transportation operations.

3. Pricing/Payment

The Client agrees to pay BMH logistic the amount of 10% per week for the dispatch services provided by BMH Logistic LLC in accordance with this Agreement.

Payment Schedule: BMH Logistic will invoice the Client on a weekly basis for services provided in the previous week. Invoices will be sent on Monday and payment is due within 3 days of the invoice date.

Payment Method: BMH logistic LLC will invoice Client as per the terms of the agreement via Email. Payment can be made to: BMH Logistic LLC by Credit Card, ACH payments, bank transfer.

Late Payment and Non-Payment: In the event that payment is not made on time or the Client fails to make payment in full, BMH Logistic reserves the right to assess a late payment fee or suspend or terminate the services provided to the Client until payment is made in full.

Disputes: In the event of a dispute regarding the payment specified in the Agreement, the parties agree to work in good faith to resolve the issue. If a resolution cannot be reached, the parties agree to submit the dispute to binding arbitration in accordance with the laws of State of Wyoming. The prevailing party in any such arbitration shall be entitled to recover its reasonable attorneys' fees and costs incurred in connection with the dispute.
